The Pathways Center provides a
comprehensive model of care that addresses the multiple needs
of HIV-AIDS patients. Using an interdisciplinary
case-management model and TEAM (Team, Evaluation And
Management) approach, our physicians, nurses, case managers,
social workers, psychiatrists, nutritionists, and substance
abuse counselors assist HIV-affected clients in coping with
the medical, financial, social, psychological and legal
matters affected by their HIV-AIDS status and accompanying
illnesses.
