Pathways Center

Comprehensive Care of AIDS and HIV Disease

The Pathways Center provides a comprehensive model of care that addresses the multiple needs of HIV-AIDS patients. Using an interdisciplinary case-management model and TEAM (Team, Evaluation And Management) approach, our physicians, nurses, case managers, social workers, psychiatrists, nutritionists, and substance abuse counselors assist HIV-affected clients in coping with the medical, financial, social, psychological and legal matters affected by their HIV-AIDS status and accompanying illnesses.

 

Services

The Pathways Center provides a wide spectrum of on-site services spanning the continuum of life-long health care needs:

  • Prenatal, Neonatal and Pediatric Care

  • Adult long-term skilled nursing care

  • Substance abuse treatment

  • Home care arrangements

  • Support groups

  • HIV education and testing

  • Insurance

  • Community Outreach

Whether serving an HIV client or counseling a family, the staff at Pathways always maintains the strictest confidentiality.

 

MEDICAL CARE

Adults with HIV-AIDS

  • Pediatric Infectious Disease Specialists provide outpatient care

PSYCHOLOGICAL AND SUPPORT SERVICES

  • Counseling and support groups for clients, their caregivers and families

  • Crisis and emergency intervention

  • Special program for HIV positive substance users with mental health issues.

MATERNAL AND CHILD HEALTH SERVICES

  • Prenatal health care for the pregnant mother, including HIV screening, testing, and counseling for the mother, children and significant others.

  • Anti-HIV treatment of the pregnant mother & unborn child

  • Obstetrical care

  • HIV counseling and testing of the postpartum mother

COMMUNITY OUTREACH

Aware that HIV affects everyone in our community, the Pathways Center offers HIV education and prevention programs to individuals and groups in our local area. In collaboration with Public Affairs, Nursing, other

St. Barnabas Hospital departments and affiliates and the Bronx HIV Care Network, we offer assistance with the following:

  • Community health fairs

  • School programs

  • Speakers for community groups
